Questions about your medical records or your care
Radiology Associates P.C. provides radiology interpretation and related services on behalf of hospitals, clinics and imaging centers. As our Privacy & Security Statement explains, questions about patient medical records – copies, amendments, restrictions and other patient privacy rights – should generally be directed to the healthcare organization where you received your care, not to us.
To report a suspected security vulnerability, privacy concern or unauthorized disclosure involving our organization, call 541.681.8586, as set out in that statement.
What this website collects
We use cookies and similar technologies to keep the site working, to measure how it is used, and to show embedded video. The full list, with what each item does and how long it lasts, is in our Cookie Policy. We do not run advertising pixels on this website, and we do not exchange your personal information for money.
Change what this site collects
That opens the same panel as the Preferences button on our cookie notice. Turning off Statistics stops the analytics cookies; strictly necessary items keep the site working and cannot be switched off. Your choice is stored in your browser, so if you use a different browser or device, or clear your cookies, you will need to set it again there.
We honor Global Privacy Control
If your browser or a browser extension sends a Global Privacy Control signal, we treat it as a valid opt-out request and act on it automatically – you do not need to do anything else on this page, and we will confirm on screen that we have honored it. You can ask what personal information this website holds about you, ask us to correct it, or ask us to delete it. Use the form below if you would rather send a request in writing, or want a record of it. We only ask for enough information to act on your request and to reply to you.
Do not include medical information in this form. It is not a secure channel for health information, and it does not reach the organization that holds your records. For anything involving your care or your results, contact the healthcare organization where you were seen.
We will acknowledge your request and respond within 45 days. If we need longer we will tell you why, and we may take up to another 45 days.
Authorized agents
You can ask someone else to make a request on your behalf. We will need written permission signed by you, and we may contact you directly to confirm you gave that permission and to verify your identity.
We will not discriminate against you
Exercising any of these rights will not lead us to deny you care or services, charge you a different price, or give you a different standard of quality. Everything on this website, and every service we offer, remains available to you exactly as before.
